Marlena brings Johnny a belated birthday present at the DiMera mansion. When she asks about his onset birthday party, he tells her he accused his wife of having an affair. He flashes to meeting Joy at the bar and adds, “I made a huge ass of myself.” Meanwhile, Chanel comes to Abe and Paulina’s apartment with a suitcase.

At the hospital, Seth tries to apologize to Stephanie for outing her and Alex’s hook-up, but only makes things worse. Meanwhile, Bonnie informs Alex at the DiMera mansion that half of Salem, including his father, knows about him and Stephanie.

At the Body and Soul office, Kate hires Joy as the newest troublemaker in Pineview: Regan Holloway.

Wearing Halloween pajamas, Leo falls asleep as he works on a script involving Reagan. In his dream, which is filled with winks at the soap opera industry, Chanel and Alex are their Body and Soul characters, Faith and Arrow. They discuss how happy they are while at the Brady Pineview Pub. As they kiss, Joy, as Regan, storms in. Upon learning Regan and Arrow are married, Faith runs out, and Regan pulls Arrow into a kiss. He breaks away, reminding her they have been over for a long time. Faith is the only woman for him. Arrow leaves to find her. Pouting, Regan picks up a steak knife from the table. “Faith, huh?” she muses. “We’ll see about that.”

A hooded and masked figure lunges a knife toward Faith

In tears, Faith rushes into her father Detective Marshall’s (Abe) apartment. Marshall assures her everything will be all right and leaves for the station. A masked figure comes to the door with a steak knife. Later, Arrow finds Faith stabbed to death on the floor. A devastated Arrow picks up the knife, wondering who could have done this to her. Marshall returns and growls, “You did.”

Arrow desperately suggests Regan is framing him for rejecting her. Johnny arrives on the scene as Detective John Hotson. Wearing a deeply open-collared shirt and gold chain, he notes Arrow looks like the Necktie Killer. “That’s not even the right storyline!” Arrow exclaims and explains he saw someone wearing a white Halloween mask. Hotson finds the killer’s mask in Arrow’s gym bag and arrests him. As Hotson brings Arrow to a jail cell, Arrow gets the jump on the detective and flees.

A handcuffed Arrow screams in an open jail cell door. He lunges toward Hotson who falls back.

Still handcuffed, Arrow races to the mansion where Bonnie, as Arrow’s mom Cassandra, reads about him killing Faith. Cassandra tells him they’re calling him the Pineview Plunger. “The Plunger?” he scoffs. “Seriously?” Cassandra agrees it needs a rewrite but also believes he’s innocent. When Arrow laments losing Faith, she reminds him that the begonia brought her back to life. Maybe it can do the same for Faith.

At the hospital, Arrow meets Cassandra, who convinces Seth’s Dr. Lamoray to bring her a dose of the begonia juice. Mother and son plan to meet in a hospital room when she gets the juice. However, the Pine View Plunger gets to Cassandra before Arrow arrives. He finds her dead on a hospital bed. He picks up the bloody knife as Hotson and Marshall appear on the scene.

Wearing hospital scrubs, Arrow stands over a stabbed Cassandra. He holds a bloody knife.

The detectives bring Arrow to the hospital lobby, where they discover Regan lying on the floor with a knife in her chest. “She’s been plunged!” Marshall exclaims. Arrow makes a break for it, and Hotson chases him.

Arrow bursts into the Body and Soul office and insists to Regan’s friend Miranda (Stephanie) that he’s being framed. She tells him that Regan vowed to get him back if he rejected her and that someone would help her do it.

Arrow confronts Dr. Lamoray at the hospital, believing Charlemagne is the Pineview Plunger. When Lamoray reminds him Charlemagne fell down an elevator shaft, Arrow declares that Charlemagne was in contact with Regan. He accuses Lamoray of bringing Charlemagne back to life with the begonia juice. Seth admits it, but he was just following orders. The plunger arrives.

Later, Arrow calls 9-1-1 as Lamoray lies dead in a hospital bed. He says the Plunger struck again and that Charlemagne is alive. She must be holed up at the Delacroix mansion. Meanwhile, Marshall and Hotson come up empty as they search the DiMera Delacroix mansion. Hattie as Charlemagne emerges from the secret tunnel.

Miranda lies draped over a Body and Soul desk with a knife in her back. A horrified Arrow stares off screen

Arrow rushes back to the B&S office and finds Miranda with a knife in her back. Of course, Hotson finds Arrow with her. Hotson’s there to corroborate Charlemagne’s story that she was with Miranda the night of the first murder. Arrow eludes Hotson and races back to the mansion. He finds Charlemagne and Marshall dead and picks up the knife. Hotson runs in panting. As they try to figure out who the killer is, the Plunger stabs Hotson in the back and stares down Arrow.

In a very leafy outdoor setting, a glamorous Cassandra wears an oversized floppy hat as she beams at Arrow

Arrow wakes up on a bench in a hazy, tropical setting. He reunites with Cassandra and discovers all the other there too. A previously killed-off Lorna (Kate) joins them and says, “Welcome to Weivenip,” which is Pineview spelled backward. As they wonder who is behind this, a smug Leo strolls in. He informs them he’s responsible for all of their lives — if they live or die, if they fall in love or break up. But the Plunger returns, surprising even Leo. They whip off their mask, revealing… Dr. Marlena Evans! She yells at Leo for bringing her back from the dead when she explicitly told him she only wanted to do one scene. She advances on him with the knife until Leo startles awake in the Salem Inn.

Leo gets out of bed and answers his door to the Plunger. Later, Leo lies dead. Stephanie tears off the Plunger mask as Kate and Abe peer over the body. “That’s what you get for writing yourself into a corner,” Kate muses. She then credits Stephanie for her great idea of killing off the head writer before the show airs. Abe agrees it will be big news. Killing an actor could anger the audience, Stephanie muses, but who cares about the writer? They cackle. “Fade to black Leo Stark,” Kate taunts.

Leo wakes up in a panic. There’s a knock at the door, but it’s just Abe who wonders if Leo got Kate’s message about the new cast member.

On the B&S hospital set, Johnny starts to apologize to Alex, who tells him to apologize to Chanel instead. Chanel joins them. Alex leaves Johnny to grovel. He asks Chanel if they can move past what happened. Kate interrupts them to introduce the latest cast member… Joy Wesley. Joy’s face falls upon seeing Johnny, who forces a smile.
